The first step in applying for a passport is to print and complete Form DS-11: Application for a U.S. Passport. Then, present your completed form to a Passport Agency or Acceptance Facility. Do not sign the application until you have received approval from an Acceptance Agent. It is essential that you enter your Social Security Number on the form. Failure to do so may result in application delays or even refusal.

Along with your application, you must provide proof of US citizenship. The following are some instances of primary and secondary citizenship evidence.

Primary proof of citizenship:

Previously issued, undamaged United States Passport Certified Birth Certificate issued by the city, county, or state Consular Report of Birth Abroad Birth Certificate Naturalization Certificate Certificate of Citizenship

If you cannot give main proof of citizenship, you must submit secondary evidence:

Census record Baptismal certificate Hospital Birth Certificate
Early school history
Bible record for the family
Doctor’s post-natal care record

When applying for a passport in person in the United States, you must additionally submit one piece of approved identification. The following are some primary and secondary identification instances.

Primary Identifier:

Naturalization certificate already issued, undamaged U.S. passport
A valid driver’s license is required.
Valid government identification
Valid military identification

If you are unable to give main identity, you must offer secondary identification as follows:

A combination of papers, such as a Social Security card, a credit card, an employee ID card, and a library card.
A witness must be present at the time of application, have known you for two years, be a United States citizen or permanent resident, have a valid ID, and fill out Form DS-71 ‘Affidavit of Identifying Witness’ in the presence of a Passport Agent.

In addition to the original document, you must produce a photocopy of each identity document. Photocopies must be made on plain white 8 1/2 by 11″ standard paper, with both the front and back of the identity document visible. Each photocopy must include photos on just one side of the paper; numerous distinct pages may be shown.

Finally, you must pay the application cost and submit a picture for your passport.
