10-Day Notice of Lease Termination 

10 DAY NOTICE OF LEASE TERMINATION

Any and All Unknown Occupants

You are hereby notified that you have broken the terms of your lease of [address] by:

[describe/list broken terms]

☐ You do not have a legal right to save your lease by fixing this violation. Because of this, your landlord has decided to end your lease, and you are hereby notified that your lease will terminate 10 days after the date you are served with this notice. You are required to move out and deliver possession of the premises to the landlord within that time. If you fail to do so, your landlord may file a lawsuit to evict you and may seek a judgment for any money you owe under the lease terms.

☐ You have a legal right to save your lease by fixing this violation, but only if you do so within 10 days after the date you are served with this notice. If you do not fix the violation in that time, your lease will terminate immediately and you will be required to move out and deliver possession of the premises to the landlord. If you fail to do so, your landlord may file a lawsuit to evict you and may seek a judgment for any money you owe under the lease terms.

If you move out within this time period, please tell us using the contact information below. Promptly informing us that you have moved out on time will prevent an eviction lawsuit from being against you.
