How to Move Medical Records

When you switch doctors or health care providers, you should provide copies of your medical records to your new provider so that they are aware of your medical history and can offer you with better treatment. Alternatively, you may simply want a copy of a specific record forwarded to someone, such as a specialist. Whatever your purpose for transferring medical information, the procedure is rather simple: just fill out a Medical information Transfer Form and submit it to your doctor, clinic, medical group, or health care institution, along with instructions on where to send the copies. The following information must be included:

Your name, as well as any other names under which your information may be submitted
Contact details (address, email, and phone number)
Your personal identification number
Your date of birth
Any details on which records will be released?
The authorisation to transfer records’ expiry date
