The Department of State provides Hague Adoption Certificates, which confirm that an adoption in the United States was conducted in conformity with the Convention and the Intercountry Adoption Act. They provide the adoption recognition in the United States and other Convention nations. After obtaining the Hague Custody Declaration, the receiving nation may approve final adoption and provide the certificate.

Fill out Form DS-5509 Application for a U.S. Hague Adoption Certificate or Custody Declaration to request a Certificate from the United States. Submit the form and any supporting documentation to the Department of State’s Office of Children’s Issues.
